There’s an option right there in the TotalTerminal settings, “Show icon in Status Menu”.
It gets greyed out when you check the option to run Terminal as a background app. I don’t know why, I don’t think it used to be like that.
I neither, just before I upgraded to Yosemite the menubar icon could be hide when TotalTerminal runs as a background app, so I think it should be an issue since we can access to TotalTerminal in several ways and not everyone loves this icon stick in menubar
